Diagnosis /Symptom: Microscopic Hematuria

What should the referring provider send?

1. A Complete urinalysis reports including microscopy (greater than 5rbcs/hpf ; should not proceed with dipstick results only)
2. Creatinine and electrolytes
3. C3 complement
4. Urine calcium/creatinine ratio
5. Report of Renal ultrasound, if obtained
6. Documentation of hypertension, if noted
7. If Lupus is clinically suspected, results of ANA, Double Stranded -DNA, ESR and CBC would be included.

Diagnosis /Symptom: Gross Hematuria

What should the referring provider send?

1. A Complete urinalysis reports (greater than 5rbcs/hpf )
2. Creatinine and electrolytes
3. C3 complement
4. Report of Renal ultrasound (if obtained)
5. Documentation of hypertension, if noted
6. If Lupus is clinically suspected, results of ANA, Double Stranded -DNA, ESR and
7. CBC would be included.

Diagnosis /Symptom: Proteinuria

What should the referring provider send?

1. Rule out postural proteinuria, if posturally related and normal serum creatinine and renal ultrasound: no referral needed (benign condition).  If non postural related (persistent): urine protein/creatinine ratio greater than 0.2 then proceed to refer and send data.
2. Creatinine
3. Serum albumin
4. Renal ultrasound without evidence of hydronephrosis
5. Documentation of hypertension, if noted

Diagnosis /Symptom: Nephrotic Syndrome

What should the referring provider send?

1. All previously obtained blood work including serum creatinine, electrolytes and albumin

Diagnosis /Symptom: Renal Tubular Acidosis  

What should the referring provider send?

1. All previously obtained blood work results including: serum electrolytes, creatinine and venous blood gasses, urinalysis  and renal ultrasound report

Diagnosis /Symptom: Hypertension

What should the referring provider send?

1. Results of previous work up including at least Basic metabolic panel, urinalysis, and report of renal ultrasound with Doppler flow.
