Diagnosis:  Disruptive Behavior Disorders (Attention Deficit Hyperactivity Disorder-ADHD, Oppositional Defiant Disorder-ODD)

What should the referring provider send?

1. Presenting problem, reason for referral
2. Consultation question, expectation regarding level/mode of feedback
3. Acute safety concerns, e.g., suicidality, aggression, psychosis, etc…
4. Current and past psychotropic meds, results, tolerance, if known
5. Current and past psychiatric history, if known
6. Current and past medical history
7. Developmental, family and social history
8. Psychological testing results if applicable and available
